Do not use this dr. He is a quack looking for money only. Charged initial visit. Then charged to find out if approved. Then only gives a three month card. I have been an MMP in NYS for years I explained to him I just needed to transfer to PA. Well $400 later and I no long have a card and refuse to give him another dime. Find a real doctor.
